A multi-end healthcare support system for older adults with chronic conditions. The core idea is to translate complex medical instructions into plain-language daily tasks, reminders, feedback, and a closed care loop.
Overview
The project addresses a common gap in chronic disease management: medical instructions are often professional, fragmented, and hard to execute in daily life. ElderCare Plain-Language AI reframes those instructions as understandable tasks that can be managed by caregivers, confirmed by older adults, and reviewed by doctors.
The system logic follows a care loop: doctor instruction, AI simplification, task conversion, reminder, patient action, feedback, and data review.

The system connects medical, caregiver, home-device, and monitoring contexts.
Caregiver App
Caregivers upload prescriptions or medical instructions, then the system recognizes text and turns it into task lists for medication, follow-up visits, refills, and health suggestions.
Medication Device
A simple screen-based reminder device shows what medicine to take, how much to take, and records completion with minimal interaction from older adults.
Doctor and Dashboard
Doctors publish instructions and review medication data, while a home dashboard makes the day's tasks visible in a familiar household setting.
